Weekly Support Groups

I provide weekly caregiver support groups, both in-person and online. Our in-person group meets every other Tuesday at St Catherine’s Church from 10:00am-11:30am, while our online Zoom session is held every other Wednesday from 9:30 am to 11:00 am. Adult Day, Home Care Services, Estate Planning/Financial Planning

Adult Day Services provide a safe, engaging environment for your loved one while giving caregivers time for work, errands, or much-needed respite. Home Care Services allow individuals with dementia to remain in the comfort of their own homes, promoting quality of life and longevity. Estate and Financial Planning are also essential, as addressing legal and financial matters early ensures your loved one’s needs are met. Here, I help families navigate these options, providing guidance and resources every step of the way.

Long Term Care/Memory Care placement

Deciding on long-term care raises many questions; costs, payment options, Medicare/Medicaid coverage, and timing. I bring both personal and professional experience to help you navigate this process. I assist with touring and selecting the right facility, ensuring your loved one maintains dignity and quality of life. Even after placement, I provide ongoing support, visiting regularly and advocating for their needs.

Case Management 

Once you reach out, Shirley will meet with caregivers to complete a comprehensive intake assessment. Understanding your loved one with dementia is key to identifying the support and services you need. Based on this assessment, I create a personalized care plan designed to meet your family’s unique needs. Our case management services provide ongoing support, monitoring your loved one throughout their dementia journey and adjusting the care plan as needed to ensure they continue to receive the best possible care.

Safety and Caregiver Resources

Ensuring safety is essential for individuals with dementia, especially as their needs change over time. I will visit your home to conduct a thorough safety assessment and provide recommendations to help keep your loved one secure. I also understand that being a caregiver can be overwhelming, and every journey is unique. Drawing on both personal and professional experience, I guide caregivers to the resources they need to care for themselves.

End of Life Conversations 

While discussing end-of-life wishes can be challenging, understanding your loved one’s preferences is essential. A person with dementia may have personal, spiritual, or cultural beliefs that influence care decisions, and these conversations may need to happen more than once as the illness progresses. Addressing these decisions before a health crisis arises ensures clarity and peace of mind. I am here to support you and your loved one in approaching these important discussions with compassion and care.
